It helps identify visible ischemic changes in patients with suspected stroke- related circulation occlusion. The software is designed to process volumes without contrast. The Alberta Stroke Program Early CT Score (ASPECTS) is a grading system that scores from 0 (most severe) to 10 (least severe) for detecting early ischemic changes on non-contrast CT brain scans. It identifies 10 regions on each hemisphere based on an atlas and calculates the average Hounsfield unit (HU) values of the brain tissue voxels in each region: • Voxels that are too dark or too bright are excluded from the calculation. These voxels may contain liquor, old infarcts, bone, and calcifications. • For regions M1 through M6, the average HU value is based mainly on gray matter. • To confirm the calculation, review the results of the automatic measurements. Image requirements: To calculate results, consider different image requirements. Mandatory image requirements: • Non-contrast CT head scans • Axial reconstruction • Matrix size: 512 x 512 px Recommended image requirements: • Slice thickness: o 5 mm without plane alignment* o ≤ 1.5 mm with plane alignment* • Smooth kernel reconstruction *The system automatically adjusts image plane alignment to compensate for patient mispositioning, ensuring standard anatomical views. Load and select a data set that meets the requirements. 2. In the Tool Gallery, click the ASPECTS icon . 3. A 2D image layout opens, and the ASPECT score calculation starts automatically. These regions are also outlined in red. • Bold text indicates regions visible in the slice. • Regions marked with WM, may have affected white matter. • Clearly visible hypodense areas (for example, old infarcts) are displayed with a thick yellow overlay. If these areas cover more than 70% of an ASPECTS region, they are excluded from the ASPECT score calculation. The corresponding HU value is replaced by a ” – “ . HU value and affectedness are marked with a dash ” – “ on the contralateral side. The HU value remains valid, but the regions are marked as non-evaluable. These regions are considered affected in the ASPECT score. You can edit the result only immediately after the evaluation with the ASPECTS plugin in MM Reading. 1. Click the region you want to edit. The edit mode activates automatically and is indicated by a ± symbol next to the mouse pointer. \ 2. If necessary, use the Hide graphics tool to hide or show the overlays. Some tools, like flipping the image, are not available. The 2D display type and image position are fixed. 3. To change the affectedness of a region, click the corresponding region. 4. To cancel your modifications and return to the original results, click the Cancel icon 0 in the mini toolbar. 5. To confirm the results, click the Confirm ASPECTS icon in the mini toolbar. The result se- ries is created. 6. If necessary, save the output image as an RGB output by right-clicking the ASPECTS icon in the Tool Gallery and selecting ASPECTS Properties.
