Navigation Hints Learning Objectives syngo 3D Registration for Needle Guidance Workflow: Define Path, Progress Needle Examination Room: Bull’s Eye View, Progression View, Needle Alignment Patient movement, Control scan, Collimated syngo DynaCT Learning Objectives Step 1/13 Control Room – Screen Capture Load CT imaging of AmoVes 200 (Abdomen with contrast) Perform VOI punching of the CT table Segment menu – Segmentation – VOI punching Remove the CT table Chose an appropriate window level in VRT Start the workflow Needle Guidance Step 1/13 Step 2/13 Control Room – Screen Capture Needle Guidance – Define Path Chose an appropriate window level for CT imaging Identify the straight predefined puncture path of the phantom in MPR (More central path leading straight to the puncture cavity of the phantom) Click on the + button to create a new path Select the entry point Select the target Verify and correct the path Needle Guidance – Progress Needle Switch to Progress Needle (syngo 2D/3D Registration will open automatically) Switch to 3D3D Step 2/13 Step 3/13 Examination Room - Screen Capture Center the phantom using the laser crosshair Prepare for syngo DynaCT acquisition Select 4s DCT Body care Auto-run – Frontal view: Central position Auto-run – Lateral view: Optimize table height Auto-run – Move to 3D test run start position Auto-run – Test run: Prevent collision Fluoro: Exposure Control syngo DynaCT acquisition [DynaCT 1] Step 3/13 Step 4/13 Control Room – Screen Capture syngo 3D Registration Click on Align volumes automatically Blend between syngo DynaCT and CT using the Dual Volume Properties dialog Verify the result in VRT and/or MPR (If automatic alignment fails, use manual alignment to align both volumes in all planes) Click on Accept 3D3D (Needle Guidance – Progress Needle will open automatically) Step 4/13 Step 5/13 Control Room – Screen Capture Optional: Check the needle path Switch back to Needle Guidance – Define Path (Needle Path Visualization can be used at any time during the use of Needle Guidance) Click on Auto Flight to start the needle path visualization (Auto Flight is helpful to recognize potential punctures of critical structures) Click on Ortho View to reset MPR plane orientations Click on In Plane View to align MPR to the puncture path Needle Guidance – Progress Needle Switch to Progress Needle Step 5/13 Step 6/13 Examination Room – Screen Capture Adjust the C-arm for Bull’s Eye View Onscreen Menu – 3D Sharing VRT – Bull’s Eye View Auto-run – Bull’s Eye View Adjust the table according to positioning graphics displayed on Live screen Adjust FOV Collimate the FOV Field of View (FOV) 20cm Fluoro (Live screen shows orthogonal view onto the puncture path. The existing channel in the phantom will result in a bright spot.) Step 6/13 Step 7A/13 Examination Room – Recording Needle positioning with the laser: Insert the needle in Bull’s Eye View Use the blunt-tip 14G needle to avoid damage to the phantom system Use paper on your needle as a screen for the laser Align needle and laser crosshair Progress the needle Step 7A/13 Step 7B/13 Examination Room – Recording Needle positioning with fluoro: Insert the needle in Bull’s Eye View Use the blunt-tip 14G needle to avoid damage to the phantom system Use a clamp to hold the needle Correct needle angulation according to fluoro Once aligned, the needle looks like a dot on fluoro Progress the needle Step 7B/13 Step 8/13 Examination Room – Recording Mimic patient movement Move the phantom slightly Step 8/13 Step 9/13 Examination Room – Screen Capture Fluoro Check for signs of patient movement: Mismatch: Needle – Needle Path Mismatch: Bone in Fluoro – Bone in VRT Adjust the C-arm to Progression View 1 Onscreen Menu – 3D Sharing VRT – Progression View 1 Auto-run – Progression View 1 Fluoro Change the working projection Rotate the C-arm to 90° RAO Fluoro (Beside Bull’s Eye and both Progression Views, Needle Guidance represents the needle path in every working projection) Step 9/13 Step 10/13 Examination Room – Screen Capture Acquire a syngo DynaCT care for 3D3D Registration Select 4s DCT Body care Auto-run – Frontal view: Central position Collimate (cranio-caudal): include the needle (center of the FOV) and two vertebral bodies Auto-run – Lateral view: Optimize table height Auto-run – Move to 3D test run start position Auto-run – Test run: Prevent collision Fluoro: Exposure Control syngo DynaCT acquisition [DynaCT 2] (syngo 3D Registration will open automatically) Step 10/13 Step11/13 Control Room – Screen Capture syngo 3D Registration Application Control – syngo 3D Registration – 3D3D Click on Automatic Registration Verify the result in VRT and/or MPR Blend between syngo DynaCT and CT using the Visualization slider Click on Accept 3D3D (Both volumes are shown in 3D Sharing. syngo DynaCT can be used to assess the actual needle tip position) Step 11/13 Step 12/13 Examination Room – Screen Capture Adjust the C-arm for Bull’s Eye View Onscreen Menu – 3D Sharing VRT – Bull’s Eye View Auto-run – Bull’s Eye View Adjust the table according to positioning graphics displayed on Live screen (As the patient has moved, the table has to be re-adjusted) Fluoro Live screen shows orthogonal view onto the puncture path (The needle should appear as a spot) Step 12/13 Step 13/13 Examination Room – Screen Capture Adjust the table for Progression View 1 Onscreen Menu – 3D Sharing VRT – Progression View 1 Auto-run – Progression View 1 Place the needle under fluoroscopic control Step 13/13 Additional Training Ideas Use the path as defined previously and put the needle in the angled puncture path. Can you identify misplacement in both progression views? Create a second path and switch between both from the examination room Use table controls on the touchscreen to select Bull’s Eye View and Progression View. Use the save C-arm position buttons to easily switch between Bull’s Eye View, Progression View 1 and Progression View 2. Define a puncture path from the examination room.
