Alpha reconstruction training Spectral reconstructions Since the introduction of Somaris 10 and the Recon and Go technology, the reconstruction power straight from the acquisition workplace has been dramatically improved. There are now new powerful reconstruction methods with no limitations on the amount of reconstructions that can be made. You now have the possibility to have packs ready post processing results done on the console and saved in the protocols. Dual energy spectral recons can now also be made straight from the console organ. Specific recons can be assigned and made for specific Recon ranges with 0 clicks from the. Operator, Now that we have covered the basic reconstructions, we will move on to the spectral recons available with the Neo Tom Alpha. These modes are not exactly new to the alpha. But have. Been expanded and are used now more. Frequently. That dual energy and spectral information is available in the quantum plus and quantum modes, the. Spectral reconstruction parameters provide basic. Grayscale results series that can be combined with all other. Reconstruction parameters Spectral recons are indicated by a colored circle in the corner of the reconstruction icon. Spectral recons can be found in the Recon and go tab. Spectral recons can be combined with fixed axial orientation and impression, multi recons and single energy inline results. Available spectral recons on the alpha include mono, energetic plus, virtual, non contrast, VNC, Iodine, Pure Calcium, Pure Lumen, and T3D. The first of these types is mono energetic plus and is available for quantum and quantum plus scan modes. Mono energetic plus images are equivalent to images with a single photon energy beam. Depending on the energy Kev different energies Kev will enhance the contrast between different materials. For instances a lower. Kev will. Show iodine contrast bright and a high Kev can help to reduce metal artifact. New to the alpha, all output images are now a mono energetic plus image unless the T3D Recon is specifically set. Even if Spectral Recon is set to none, the reconstruction series will still be a mono energetic plus image. The default Kev level of the series is determined by the KRKEV optimized for setting for the scan range. For instance, the non contrast setting will have a 70 Kev Recon for all default reconstructions. If a different Kev level is desired, then you would choose a mono energetic plus reconstruction in the Spectral Recon tab and choose your desired level. Here is an example of a mono energetic plus image. On the left is a mono plus 70 Kev and is comparable in noise and contrast to a classic 120 kvp image. On the right is a mono plus 45 Kev image. You can see that the contrast is bright, but the noise level is a bit higher than the 70. The next spectral Recon is virtual non contrast or VNC and is available only if the scan mode Quantum Plus was used. In a virtual non contrast image the iodine contrast is removed from the image and is not visible. This can assist in interpretation if a noncontrast scan was not performed and the physician needs to see some pathology where the contrast is not present. Here you can see an example of a virtual noncontrast image. On the left side you can see a mono plus 70K EV image and on the right is the VNC where the iodine contrast has been completely subtracted from the image. After the VNC, the next spectral Recon is iodine. Again, this is available for Quantum Plus scan modes only. In an iodine series, the images show the concentration or enhancement of the iodine after a contrast examination. This can be of use if the physician needs to see if there is any iodine uptake in certain pathology. With an iodine reconstruction, the results series will be in grayscale. Here you can see an example of an iodine image. On the left is the mono plus 70K EV image and on the right is an iodine map image. Notice all the CT data is removed from the iodine map where only the iodine and the calcium of the bones remain in the grayscaled image. The next three spectral recons are new reconstructions and are exclusive to the neo Tom alpha. The 1st is. Pure calcium. And is available in quantum plus scan modes only. Pure calcium is a series where the iodine has been subtracted and the calcium in the vessels is left behind in the images. The resulting images are equivalent to true non contrast images and can be used for calcium score even after a CTA examination. Keep in mind that A70K EV and a QR36 kernel reconstruction needs to be used for scoring purposes. All other Recon parameters should match that of a traditional calcium score i.e., field of view, slice thickness, and windowing for the optimal results. Automatic calcium scoring with inline results is not currently available. Here you can see an example of a pure calcium image. On the left is the. Cardiac CTA examination with. 60K EV. And on the right is a pure calcium image. Notice all the iodine has been subtracted from the image on the right and the calcium has been left behind for calcium scoring purposes. These two images are from the same examination. Next is the pure lumen reconstruction, which is again only available with the QUANTUM plus scan mode. The pure lumen series is generally used in a CTA examination. And is utilized to remove any calcium from the vessels in the output series. Removing the calcium is helpful to see the actual lumen of the vessel without the blooming artifact from the calcium. This way the physician is able to see if there is a stenosis behind the calcium that before he was unable to. See because. Of the artifact caused from calcium the output. Series is a mono energetic. Series as. Well, with just the calcium removed. You can freely select the output. Kev level of the series having different energies that can enhance the contrast. In the images between. Different materials can improve the visualization in the vessel. Here again is an example of the pure lumen reconstructions. On the left again is a mono plus 70K EV image and on the right is a pure lumen reconstruction with the calcium removed from the coronary artery. With pure lumen you are now able to see the lumen of the vessel, whereas before it is not possible due to the calcium blooming artifact. The last spectral option is T3D and again is exclusive to the alpha due to its. Photon counting detectors, all image series will be a mono energetic plus series with the exception of AT3D series. T3D. Is available for the Quantum and Quantum Plus scan modes. The T3D reconstruction provides images generated from all photons registered at the. Lowest energy threshold. And are the look of a classical CT image similar to an image from a traditional CT detector system. This Recon could be used for fast reconstructions for perfusion imaging and cardiac multiphase since these. Typically have. Thousands of reconstructions. AT3D is the equivalent to a reconstruction from a CT scanner with energy integrating detectors.
