syngo.via Frontier Cardiac Functional Analysis

This demo video explains how to perform a cardiac functional analysis with syngo.via Frontier.

In this research application, we are able to perform a quantitative statistical analysis of 2D polar map with the related American Heart Association segments where we can draw region of interest which also measures the underlying 3D data. This relates in particular to dynamic myocardial perfusion analysis studies. This research application is also suitable for measuring the extracellular volume. It's suitable for both single energy data and for dual energy data. To open the case, please right click on the selected patient, select open with and then select CT cardiac function analysis frontier. This data set is sent to a single via frontier. Then we will be able to select the research application that we want to have. Here we have the list with research applications that can be selected. We can select cardiac functional analysis for example. It is always good to check if you have the latest version of this research application. Research applications are improved locally every now and then and this is version 24. We also have version 23 available over here, but we will select this one. Behind the information button you can find references, but also the manual for this research application. It's always useful to check the manual if there have been changes in the research application. Once we click on the tile, the research application starts and prepares to show us the images. Once a research application is opened, a screen appears where we can mention which different data roles should be assigned to the different series we have. Here is a drop down menu where you can select what kind of a series. The system automatically detects it already. This is the VPC Act average data set. But if the recognition is not done automatically, you can simply select the possibilities here. If you can change the series names, it might be nice to change the names accordingly to one of these tags. The system will automatically start and find the data rolls for the different series. This is the average. We leave it like this and simply press OK to start the analysis. I selected the myocardium with the EPICARDIUM and the endocardium lines and if the system does not work properly, most of the time it does because it is very well programmed, then we can select this ourselves. We need to drop down this menu and press edit contours. Afterwards you will find a little red area over here or we can select the blue area. You see that we simply can draw a Alongside myocardium. Now we connect to the blue line again and DoubleClick. And then you see that the line is updated a little bit. To switch off this functionality, press on this button again. We can also initialize the segmentation that the system did manually. It's useful when the system does not work properly. Furthermore, we can edit the base plane. In the base plane, we can scroll this a little bit up because everything below the base plane will be taken into account in the measurements. It was a little bit low. We have here a button for the different lanes that we have. Or we even have a button for the orientation. We can have a mitral valve orientation or an aortic valve presentation over here. When we window a little bit we can see this all a bit better over here, and the aorta valve is quite nicely presented. We go over to the analysis tab. Here we will do the left ventricle analysis. We have over here the first pass enhancement. I will change the view again back to the two Chamber view. The four chamber view and the longer axis that we have over here. In the meantime, the system already did some measurements. That can be very useful. And with color coding, the system offers this functionality over here, the first pass enhancement in the myocardium. With this slider bar that we can find here, we can set the sensitivity. It's now automatically set to 83, which gives us a nice view of this first pass enhancement. The next step that we can do with this research application is to analyze the polar maps. Once I click on the button, the compute polar maps will be calculated and will be presented to us. The polar maps are presented here. We can double click this segment to make this a little bit bigger and you see quite clearly the American Heart Association segments with the different measurements. We can select the measurement that we want to have. For instance, the VCT blood volume, maximum enhancement total and a blood flow for the endocardium. Here we can measure in those areas. You see qualitatively in color coding the values and you can also see some values when you hover over it with the mouse. To make this a little bit more precise, we can do a circular region of interest measurement. And for instance, in this area we can measure this region of interest and get the numbers in milliliter per 100 milliliter tissue per minute. As a comparison, we can take a segment at 10 which is apparently not affected as a reference to compare those values. Later this data will be sent out so we can analyze this in a different way. The region of interest is still active, so we deactivate it. I double click again. We can do some polar map navigation. It is assigned to the endocardium. You can see it over here. The endocardium or epicardium measurements are set over here, so we can select those possibilities. What we can also do is put polar maps here and integrate them in the areas that we find over here. I will switch off some functionalities at this point. And then we get the grayscale, for instance the endocardium mask over here. You see the blood flow color map. It's quite clearly visible what we can do with this. We like to send this out. Because when we alone look at this data, it doesn't make so much sense. Therefore we have different data exchange possibilities. We can also load a data set in here. And even load a region of interest. We can send out those data as an STL file. I'll show this to you. We store this on the desktop. We can export the different regions of interest. And we can also export this according to the AHA results over here. Let's say we export the region of interest from the polar maps also to the desktop. This research application is also available for the analysis of the extracellular volume. For that you need to load different data which are not currently available in this research application. You would have to leave select the data and select the CV analysis again. We will now go to the desktop of Syngo via. Here we have a cardiac functional folder over here. Let's see what was given to us. We have the MPI which might be the STL file. But when we look at the properties, we clearly see that it is a STL file. We can use this for 3D printers. We have here the AA mapping in a comma separated value. When I open it in the preview lane, you can see the note with the CSV values here, where we have all the measurements available with the numbering. Afterwards we can easily enter this into an Excel sheet to find out the different measurements. In a comma separated value file we have the different regions of interest which we can do further analysis on. Of course, this research application has also the help functionality, a reset button. And the possibility to take screenshots of it, which we also can send to the desktop for later analysis. And with that, I thank you very much for your attention.
