Introduction to Direct i4D Workflow [Audio file name: audf_som_go_open_pro_direct_i4d_wf_olt_intro.mp3] Our solution consists of 3 main steps: The first step is the selection of the correct scan parameters based on the breathing pattern prior to the scan. We call this FAST 4D. In the second step, Direct i4D adapts the scan duration to the patient’s breathing pattern in real time. This is a sequential scan. And in the third step, the intelligence of Direct i4D chooses the optimal data for 4D CT reconstruction. Direct i4D Workflow Steps In this chapter, we will guide you through the Direct i4D workflow. Select the numbered steps below to learn more about it. Please note: The following steps describe the pure workflow without any configuration. Please ask your Application Specialist about the configuration of Direct i4D. 1 1 1 2 2 2 3 3 3 4 4 4 5 5 5 6 6 6 7 7 7 8 8 8 9 9 9 Direct i4D Direct i4D monitors the breathing throughout acquisition and reconstruction and is the first 4D CT sequence that intelligently adapts to the patient’s breathing in real time. ? Direct i4D Workflow Steps [Audio file name: audf_som_go_open_pro_direct_i4d_wf_olt_steps.mp3] In this chapter, we will guide you through the Direct i4D workflow. Select the numbered steps below to learn more about it. Please note: The following steps describe the pure workflow without any configuration. Please ask your Application Specialist about the configuration of Direct i4D. Step 9: Finalize the Examination 9 You can finalize an examination at any time, even if some tasks are not yet completed. In the taskflow control, select the Close task. The Job Status tab opens. It provides an overview of all recon jobs, their status, and auto tasking settings like Auto View&GO or Auto Printing. If applicable, check the Auto Tasking parameters. In the execution control, click the Close button. The patient is closed. All pending recon jobs are processed in the background. After closing the patient, the Scheduler will open automatically. Now you can select the next patient to continue with the next examination. Step 8: Apply Image Grid and Cine Mode 8 Only in Sim&GO we offer the Image Grid and the cine mode to get a first impression of the tumor movement. Proceed as follows: Once the 4D data is loaded to Sim&GO, you can open the Image Grid by clicking the corner menu at the top right. To open the cine mode, from the Series panel, load the desired phase of the study by dragging it with the right mouse button. Then select Replace As 4D. A movie player opens where you can start the movie or jump forward and backward between the single phases. Step 7: Perform Patient Marking 7 Once the images are reconstructed and loaded to CT View&GO, select the data and click Sim&GO in the toolbar area to load them into the workflow. After the data is loaded to Sim&GO, create the isocenter in the region of interest and click Set To Crosshair. Click Mark Patient. The laser will reach the Z- and X-position. For the Y-position, a move request for the table will appear at the bottom of the console. Once the RT Laser and the table are in defined position, all values will become green and you can mark the patient's skin physically. After the patient is marked, click the Marked checkmark and you can finalize the examination at the acquisition workplace or on the tablet. Step 6: Perform Image Reconstruction 6 After a scan is completed and the image quality is approved, you can proceed to the Reconstruction task. The reconstruction parameters are preset in the scan protocol, but they can still be adapted: In the Recon Ranges section, double-click a recon icon of a recon job to open the Parameter panel and edit the presets. Select Recon All in the execution control area to start all reconstruction jobs previously configured with Recon&GO. Select Next to continue with Patient Marking or to close the step. Reconstruction Possibility to perform series splitting (= multi-phase splitting, turned on by default for the multi-phase reconstruction) Possibility to reconstruct Average, t-MaxIP, t-MinIP or single phase reconstruction DirectORGANS can only be applied with Average, t-MaxIP, t-MinIP or single phase reconstruction Step 5: Check Image Quality Upon receival of the images, the Check&GO algorithm is activated. The algorithm checks the anatomical coverage based on the images and the uniform and adequate distribution of contrast media if applied. The quality check stops all auto reconstructions until the quality is approved by you. You can view the coverage or image quality of the scan by right-clicking and moving the mouse up and down to scroll through the displayed images. 5 Step 4: Perform Tomogram Scan 4 To perform the tomogram scan, proceed as follows: Define and adapt the scan range in the topogram segment. Select the FAST 4D button on the Execution Control. On the Control Box next to you, press the Move table key. The FAST 4D evaluation starts. Click GO on the Execution Control. The scan parameters are being loaded. Then press the Start key on the Control Box next to you to start the tomogram scan. With starting the tomogram scan, the patient’s breathing curve will be shaded in green. Fast 4D The FAST 4D workflow automatically adapts the scan parameters rotation time and pitch of the scan based on the actual patient breathing rate. Direct i4D is a sequential scan mode. It adapts the scan duration to the patient breathing pattern in real time. The scan takes place for one complete breathing cycle. If the system recognizes a breathing cycle which is too shallow, it waits until an adequate breathing cycle is achieved and scans until it has acquired enough data. Step 3: Perform Topogram Scan 3 Check the patient position and select Confirm to continue. Select GO to activate the topogram scan, then press the highlighted Start button on your control box next to you to start the topogram scan. The topogram is displayed in real time in the topo segment and can be suspended at any time if its length goes beyond the necessary anatomy. FAST Planning automatically places the correct recon range for you. If you wish to make additional adjustments to the recon range, grab any of the white dots and modify the recon range accordingly. Prerequisite: DirectORGANS structure template is assigned. To start the lung examination, click the Thorax as desired body region on the schematic body map or the RTP folder. All available scan protocols are displayed on the list to the right. Select the RTP Respiratory Motion Management [factory] scan protocol. Select Add. Then select OK to apply the protocol. Step 2: Select Scan Protocol 2 Step 1: Select Patient from Patient Browser On the Home screen, select Examination to perform a CT examination. System connected to a RIS: All scheduled patients are listed in the Scheduler of the Patient Browser. Select the patient and the patient data will be displayed on the Patient Registration dialog on the right side of the Patient Browser. System not connected to a RIS: Patient data can be entered and saved here manually. Check the patient data and medical information. Select Exam to load the selected patient. 1 ? Direct i4D Mobile Workflow: Breathing Curve on Tablet As part of the Scan&GO workflow, you can observe the breathing curve of the patient and the RGS name on the tablet. The breathing curve can be displayed without the need to select a scan protocol or patient. This helps to get an impression of the patient breathing.
