Somaris 7 CT Interventional workflow job aid SOMATOM Definition systems with Intervention package or 3D Intervention Suite This job aid is designed as a learning tool to guide you through a basic CT intervention workflow on the SOMATOM Definition family scanners. Protocol Selection Three scan modes are available in the intervention mode: 1. i-Spiral • Provides most z-axis coverage. Topogram • Is best for complex needle angles. i-Spiral • MPR segments can be generated automatically. • Used for localizer scan. 2. i-Sequence: Topogram • Lowest dose i-Sequence • i-Needle Sharp and HandCARE™ available. • Limited range MPR segments can be generated automatically. Topogram 0 3. i-Fluoro i-Fluoro • Highest dose. • Dynamic imaging. • HandCARE available. Preparation 1. Place the patient in the proper position, per the radiologist’s request. 2. In the Patient Model Dialog, ensure the patient orientation matches the actual patient posi- tion on the table. 3. Isocenter the patient in the standard manner. 4. Select the desired Intervention protocol for the appropriate body region. 5. Select the appropriate side for the intervention monitor. This ensures the correct orientation of the patient images (head/center/foot). Acquire the topogram. 2. Perform the i-Spiral range to the area of interest with an open FoV. SP 2: LEN: FOV ON 3. Scroll through the axial images until the desired target is located. 4. Distance can then be measured by opening the 3D Intervention Toolbar and selecting the ruler. 5. Place the cursor on the target, hold the LMB (left mouse button) down, and drag it to the entry point. With the desired axial target image still selected (blue border), select the table position drop-down and select the Same TP option. This will move the target skin entry point to the center of the gantry and in line with the transverse laser light. Move Out Same TP Last SP ht -1698.0 125.0 7. Save as Table Position 1 on the toolbar. 8. Save as a Key Image if desired from the toolbar. Note: Can drag and drop from the browser into an open segment on 2nd monitor, if available, for reference. 9. Switch to the i-Sequence mode for the lowest dose and select Move scan range to dis- played table position. 10. Make any desired adjustments to Zoom/Pan. 11. Load scan. With the table at the correct position, turn on the laser and mark the skin entry point. 2. Move the table out to a comfortable working position and save as Table Position 2 on the i-Control. 3. The radiologist will begin the procedure by prepping the insertion area, administering local anesthetic, and placing the needle. 4. Return to the desired scan location by using the Move to Saved Position 1 on i-Control or un- der the Routine tab Table > Position > Saved TP. Note: If the patient is in the proper position, and a Move Table command is present, use the Cancel Table Move button. This will allow the scan to be performed at the current table po- sition. 5. A sterile drape cover may be used to allow the performing physician to use i-Control. 6. HandCARE or i-Precision View can be activated if desired. Activate HandCARE based upon needle entry point (10, 12, or 2 o’clock) to • reduce exposure to the radiologist’s hand. Perform subsequent scans and correct the needle path toward the target. Post-procedure 1. When the procedure has been completed, and a post-procedure scan is desired, return to i- Spiral and adjust the scan range. 2. When all imaging is completed, select Cancel to exit intervention mode and end the exam. 3. Go to Patient > Browser > Local Database. 4. Select the patient and desired images to transfer to PACS.
