Welcome to the training for Neotom Alpha VB10 software. Upgrade for. Cardiac Scanning In this training you will learn about the new scan protocols, the new scan mode options, the new reconstruction options, and the new quantum HD cardiac scan modes. We will start with the new scan protocols included in the VB 10. Software. 1st. The default. Coronary CTA protocols have added a lateral. Topogram. This is to ensure isocentering of the heart is done instead of the. Thorax. To make this easier. For you. Fast positioning is now available. With these protocols and 2nd a new protocol, lung screening and coronary calcium scoring is now also available. Now let's discuss these in more detail. In all coronary CTA protocols, the default factory protocol will now include a lateral toprogram after the AP toprogram. Keep in mind this is for all coronary. CTA protocols. To include coronary CTA, coronary CTA function and coronary CTA stent. The lateral toprogram is used to isicenter the heart. This is to ensure proper image quality is acquired in cardiac scans. To make this easy for you, fast positioning has been added to this protocol. This means that after the lateral toprogram is done, you will get a move request automatically. Pressing the move button on the control unit will adjust the table height to ensure the heart will be ISA centered. As you can see in this example, the heart is now in the ISA Center. With the VB10 software. A new protocol, lung screening and coronary calcium scoring has been added. This protocol is an ECG gated flash scan and has the feature Flash Face spiral applied. Since this scan is a screening exam, it combines the established Tin mode for lung screening and also a mono energetic plus 70 Kev reconstruction for calcium score evaluation. This exam is one flash scan that you would typically do for lung screen, but now we cardiac gate this exam which allows you to do a calcium score in the same scan. This protocol is in the thorax section of the protocol selection. This new protocol you will set up as a flash chest, but now with a cardiac reconstruction over the heart in this example. You can see that the scan is only a half a second scan time. This protocol. Utilizes the quantum tin scan. Mode to keep the. Dose very low while maintaining a high image quality for screening and calcium scoring purposes. You also have the flash phase spiral feature in this protocol. This is used to ensure that the. Flash scan will be timed and gated to the heartbeat of the patient. The line in the top program lets you set where you would like the phase start to be in the physio scan. Option. You may also change the phase you wish the heart to be in. The default is set to 65% for all calcium scoring needs. As you can see in the reconstructions, full chest and lung recons are available for screening purposes of the lungs. Lung CAD is also. Available for. Detection of lung nodules. There is also a reconstruction for automatic calcium scoring of the coronary vessels. Now let's discuss the new scan mode options for cardiac scanning. There is a new scan mode in the list of available cardiac scan modes. This new. Scan mode is for retrospective spiral scans and has the keyword elevated in the scan mode. Name this scan mode. Is available. For both quantum plus and quantum scans. This scan mode will increase the applied radiation dose during the low dose plateau from 10% of the dose to 25%. In the elevated scan mode, the low dose plateau will now be increased to 25% of the applied radiation during the pulsing. This will now improve the overall image quality of the phases that are in the low dose plateau areas. If you wish to not use 25% then you can select the scan mode without the keyword elevated and have the lower 10% DOS applied. Looking at the ECG trace, the section on the left is the non elevated standard scan mode and has a lower plateau shown. The section on the right is the elevated scan mode and has a slightly higher green plateau. Let's now discuss the new reconstruction options with the upgrade. To VB10 software customers with cardiac packages. Will have. The ability for a new reconstruction technique. Called Z Free cardiac imaging is challenging and misalignments may occur. They occur due to improper acquisition or patient breathing. Often they are harmless and just aesthetically unpleasant. In very minor cases they can also affect diagnosis if the misalignment is right at the stenosis level and a true stack Recon is not available. Zfree is a. Revolutionary reconstruction feature which automatically delivers cardiac CT scans without stack misalignments originating from breathing or patient movement. This feature works independent of the detector width of the acquired data. Zfree is a detector with independent cardiac reconstruction feature it. Allows the reconstruction of ECG gated. Spiral or. ECG triggered sequence data with alignment of stacks originating from separate cardiac cycles or patient breathing. This is realized by the reconstruction of separate stacks of images for each contributing cardiac cycle, followed by a non rigid registration step. This step is based on the available overlapping image data between neighboring cardiac cycles. This feature is a Scanner integrated reconstruction option which you can manually enable. Or disable if you were to compare a standard, a true stack and AZ free reconstruction from an exam with patient motion or breathing misalignment, a standard could be blurry at the edges of the stacks leading to less clinical confidence, especially if the stack arises at a point of stenosis. In a true stack reconstruction, each stack is shown in full coverage and the anatomy is repeated 20% in each stack. You can still make a diagnosis, but reading is more cumbersome. With Z Free, images are aligned optimally to demonstrate the morphology as intended. Z Free works independently from the number of stacks or detector width. Let's now look at some actual cardiac images. Since Z Free is available on also Maris 10 cardiac capable scanners, we have this data set from the new Pro Pulse dual source scanner in the standard Recon and the Truestack. You can see the misalignment from Patient Motion. Now you see the Z Free reconstruction and the benefits of this revolutionary reconstruction feature without the misalignment. Here is one more example of how Z Free can improve the ease and accuracy of reading in cardiac imaging, all while being independent from the physical detector width of the acquired data. To recap, Z Free is a scanner integrated reconstruction feature which reduces the stack misalignments. This feature is independent from the physical detector width of the acquired data. Z Free is available for both. Perspective sequence and retrospective spiral scans. You can apply it to. All Recon types that is best phase, single phase or multi phase reconstructions. You can find Z free in Physio Recon under the Stack Recon option. The last section to discuss is the new Quantum HD Cardiac scan modes. This new scan mode is for Quantum HD Cardiac and therefore is only available in the coronary CTA stent protocol. This new scan mode now gives full spectral capabilities to the Quantum HD cardiac scans. This means that you can have cardiac scans with the ultra high resolution reconstructions of 0.2mm. Only now in addition, you can have spectral reconstructions as well, such as a pure lumen or an. SPP. This scan mode is identified with the keyword RESEARCH at the end of the scan mode name. The scan mode without the. Keyword research. Will continue to be a non spectral scan since. This new scan mode is spectral. It is only available in Quantum Plus scan modes. This new fully spectral quantum HD cardiac scan mode uses the collimation of 96 by 0.2mm keep. In mind that for all spectral reconstructions such as. Pure lumen VNCSPP that these reconstructions will need to have a slice thickness no smaller than 0.4mm and a kernel sharpness 76 and below all. Quantum HD reconstructions such as a 0.2mm slice thickness or kernels above a 76 sharpness will need to. Be at. 3D Reconstruction.
