This job aid provides guidance using MR Neurology workflow to display and evaluate perfusion-weighted images. Calculate comparative values (mismatch) between the Diffusion ROI and the Perfusion ROI to help quantify the penumbra region.

MR Perfusion Mismatch – Numaris X MR Perfusion Mismatch – Numaris X The MR Neurology workflow displays and evaluates Perfusion-Weighted images and calculates the comparative values (mismatch) between the Diffusion ROI and the Perfusion ROI. The mismatch result may help quantify the penumbra region. Note: MR Perfusion datasets must either use distortion-correction or non-distortion correction image data, you cannot combine both. 1. From the Patient worklist, load the Perfusion data set into the MR Neurology workflow. a. Right-click on the examination and select Open with > MR Neurology. Draw an ROI around the Diffusion abnormality in the upper left segment. (i.e., ROIs drawn on an image are displayed in all image segments in turquoise). a. ROI’s can be drawn on a single or multiple images. b. Multiple ROIs can be drawn on single or multiple images. ADC_MAP 6. In the lower left segment, select a Mismatch Evaluation ROI. a. Select MMEval Ellipse ROI (or) MMEval Freehand ROI in the upper right corner menu. (i.e., if the tool is already activated, you do not need to select it again). b. Draw an ROI around the Perfusion Area in the lower left segment. (i.e., ROIs drawn in the Perfusion area are displayed in Yellow). perfusion_F13_MOC Info: The mismatch calculation uses the ROIs of the upper and lower left segments, usually, diffusion and perfusion data. You can also load other image data in the left segments and create a mismatch evaluation of this data. (e.g., Dark Fluid TIRM and Diffusion data). Mismatch Evaluation Table. a. In the lower right segment, ROI’s and mismatch results are displayed in a table. b. The row marked with “Comb” combines the measurement results across all slice positions. 8. Select the Save Finding button. a. Findings are created for the mismatch evaluation. Info: If you modify a measurement afterwards, the Save Finding button changes to an Update Finding button. Use this button to update the existing finding with the updated measurement. 1. To create a new finding, click the close button in the Save/update Finding window: a. Choose Yes – create or update a new finding b. Choose No – deletes the findings and ROI’s 2. Select Mismatch Eval to start over again. 9. Export to CSV button (Optional step). a. Export results as a CSV file (comma-separated values file). Info: If needed, load further image data in the upper right segment and use these segments as a reference. This may help to find the correct positions of the diffusion and perfusion ROI’s. Info: If needed, select further slices, and proceed with the mismatch evaluation as described above as described. 10. Select the Close button. a. Exits mismatch evaluation and closes the layout. 11. Save Results. Save and Send: Results are stored on the system and sent to your archive. Save and Pause: Results are stored on the system.
