This course will provide an overview of the basic principles of IHE, its documentation lifecycle and domains. Basic knowledge of IHE will provide a solid foundation for other product training and assist you in any customer conversations you may have. IHE Definition Integrating the Healthcare Enterprise (IHE) International initiative founded in 1997 to improve the information exchange between IT systems in healthcare. Information systems that support IHE profiles communicate better, are implemented easier, and enable healthcare institutions to use their information more efficiently. Objective: Enable optimized and efficient patient care by supporting information flow across the entire healthcare domain. Some say that IHE is another language … IHE Definition What is IHE? IHE is the abbreviation for integrating the healthcare enterprise. It’s an international initiative founded in 1997 by vendors and users aiming to improve the exchange between IT systems in health care. Information systems that support IHE profiles communicate better, are implemented easier and enable healthcare institutions to use their information more efficiently. The objective of IHE is to enable optimized and sufficient patient care by supporting the information flow across the entire health care domain. Some say that IHE is another language and you will probably understand why they say that as you continue in this training. IHE Documentation Lifecycle Shown here is a representation of the IHE documentation lifecycle. IHE selects standards from organizations such as ISO, HL7, W3C or DICOM. These standards are then optimized by IHE and used for technical frameworks. Here, IHE defines use cases in the form of integration profiles. These use cases, in the integration profiles, are then organized and published for testing at connectathons. Vendors can then take the technical frameworks, develop those use cases in their systems, test them at connectathons and submit their results to the IHE product registry. Developers or vendors of health IT solutions can take these elaborated technical frameworks from IHE to implement them in their solution or use those technical frameworks in tenders. Users or purchasers can search the IHE product registry for available, interoperable products, which cover a certain use case or meet their needs. Select each plus sign to learn more about the key terminology that is part of the IHE documentation lifecycle. START TRIGGER TO NEXT SLIDE NOT LAYER Interoperability Showcase Here are some additional terms you should familiarize yourself with. An IHE Showcase is an event where vendors can show the interoperability of their products to the market. Usually IHE showcases are organized by IHE at big fairs - have a look at the picture, it gives you an impression about the HIMMS in the United States from 2017. In Interoperability showcases, healthcare stakeholders come together to demonstrate the advantages of using standards-based interoperable health IT solutions for secure and effective exchange of health data information. The IHE Conformity Assessment is offering an internationally recognized compliance of a product to one or more IHE profiles. With the Product Conformity Test Report, IHE delivers the substance that could enable certification. The certification is done by independent authorities like Concert for North America or Kereval in Europe. IHE Product Registry is a web-based database for integration statements. It enables vendors and developers of healthcare IT systems to create, manage and publish Integration statements. The IHE product registry allows you to search products from different health IT vendors which support IHE profiles. If you are familiar with the basic IHE terms, such as IHE actors, integration profiles or IHE technical framework, you can use Integration Statements to determine what level of integration a vendor claims its product and what clinical and operational benefit such integration might provide. Select start to view how to navigate to the IHE product registry and search for an integration statement. Screen 1 Navigate to the IHE Product Registry landing page by either doing a search for the IHE Product Registry or copying and pasting the URL. On the landing page, select IHE Product Registry to get started. Screen 2 On the product registry landing page select the link Click here to enter the Product Registry. Screen 3 The IHE Product Registry allows you to register and search for products supporting IHE Profiles. Select Introduction to view more information about IHE Integration Statements. Screen 4 Then select Search Integration Statements to search for integration statements. Screen 5 This is the search mask of the IHE product registry. On this page, you can view all of the available integration statements or to perform a faster search, enter more specific search criteria. Screen 6 In this example, we have just completed a search for ITH icoserve technology for healthcare by entering search text in the organization field. Our search located two results. ITH icoserve (pronounced eye-coh-serve) is a provider of software solutions in the healthcare sector and is a subsidiary of Siemens Healthineers. The products of ITH icoserve meet the relevant IHE specifications for health networking and are tested regularly at Connectathons organized by IHE. The results of these Connectathons are published by the IHE. Let’s view the Integration Statement for eHealth Solutions. To view the integration statement, select View for the specific statement you are interested in. Integration Statement Familiarize yourself with the information that is provided in an integration statement. Refer to the IHE Product Registry and search for the Integration Statement for eHealth Solutions. https://www.ihe.net/testing/ihe_product_registry/ Blank Now that you have seen how to locate these documents, when you have a moment, refer to the IHE Product Registry and have a look at the Integration Statement of eHealth Solutions. You should familiarize yourself with the information provided in such an Integration statement. IHE Domains (Technical Frameworks) Future Domains Pathology and Laboratory Medicine Eye Care Patient Care Devices Radiation Oncology Radiology Laboratory Patient Care Coordination Cardiology IHE IT Infrastructure Anatomic Pathology Dental Endoscopy Pharmacy Quality, Research and Public Health Highlight Radiology, Lab, Pathology when mention in audio IHE Domains Now that we have reviewed the basics of IHE, let’s take a closer look at the IHE domains. IHE is organized by clinical and operational domains. IHE domains represent those various clinical fields, which each develop its own Technical Framework and Integration Profiles. To mention some of these IHE domains: radiology, laboratory, pathology, oncology and so on. The initial success of IHE was achieved in radiology and is still one of the main drivers for development in health IT. One IHE domain is of interest for us, as we will learn more about it in the later chapters of this eLearning. It’s the IHE IT infrastructure, which addresses the implementation of standards-based interoperability solutions to improve information sharing, workflow and patient care. Siemens Healthineers eHealth solutions, by the way, is built on the IHE IT Infrastructure. IHE IT Infrastructure (Example Profiles) Cross-Enterprise Document Sharing Registration, distribution, and access across health enterprises of clinical documents, forming a distributed patient electronic health record Patient Identifier Cross-referencing for MPI Map patient identifiers across independent identification domains Retrieve Information for Display Access a patient’s clinical information and documents in a format ready to be presented to the requesting user Audit Trail & Node Authentication Centralized privacy audit trail and node-to-node authentication to create a secured domain Consistent Time Coordinate time across networked systems ... Patient Synchronized Applications Synchronize multiple applications on a desktop to the same patient Cross-Enterprise User Assertion Provide users a single name and centralized authentication process across all systems Patient Demographics Query IHE IT Infrastructure IHE IT infrastructure consists of 15 integration profiles. IHE integration profiles are descriptions of clinical information and workflow needs and specify the actors and transactions required to address those use cases. Integration profiles offer a way for health IT vendors and users to reference functionality defined in the IHE technical framework. The core integration profile in this framework and shown here is the profile Cross-Enterprise Document Sharing (XDS), which addresses the distribution of documents between multiple healthcare institutions. The patient identifier cross-referencing for MPI is also an important topic, as it supports the cross-referencing of patient identifiers from multiple Patient Identifier Domains. Have a look at the other integration profiles shown here. We will learn more about these profiles in later modules.
