Invasive ductal carcinoma: DBT is excellent for the detection of distortions ans spiculated lesions

Case number 12, a 52 year old lady with no symptoms, very dense breasts and the only finding on mammography were some tiny microcalcifications on the left breast. We detected the micro calcifications and a lot of cysts. You can see the micro calcifications are very clearly detected on tomosynthesis suspicious micro calcifications into ductal calcifications and it was not very clear if there was something irregular, a possible irregular mass very close to the microcalcifications. Enhanced mammography we detected an uptake of contrast in the mass that was not detected on the conventional mammography, the micro classifications only on conventional and the mass the enhancing mass close to the micro classifications. So this patient had two cancers, 1 intraductal cancer that was the area of the microcalcifications missing in five millimetres and an invasive ductal cancer, A12 millimetre cancer in the irregular mass. So it is important to know that contrast enhancing mammography can detect additional masses, additional cancers not detected on conventional mammography. But on the opposite, some tiny introductory cancers do not enhance on contrast mammography.
