A short review regarding the terminology and technology: Volume transducers are the 9VE4 and 7VC2, which are motorized mechanical transducers that performs 3D imaging and real time 4D gynecological imaging. During a 3D acquisition, a single mechanical (automated) sweep is performed and produces 3 orthogonal dimensions, or planes, as well as a volume rendering. During a 4D acquisition the transducer continuously acquires and displays the volume information. This method is also known as real-time 3D ultrasound where time is considered the fourth dimension. Healthineers ... 3D Technology 4D Technology 3 Unrestricted © Siemens Healthineers 2024 Speaker Notes: Let’s start by understanding the differences between 3D and 4D technologies. 3D ultrasound is the sequential stacking of B mode and color slices to form a volume of data – giving spatial height, width and depth perception to ultrasound data. It can be acquired in two ways, freehand and mechanically. • Freehand 3D ultrasound is when the user is moving the transducer and the sound beam in a rocked or linear motion to acquire the 3D volume. Transducers that have the 3D Freehand technology for gynecology are: 10EV3 and 9EC4 • Mechanical 3D ultrasound is when the transducer motor moves the crystal and sound beam in a single sweep to acquire the volume data. The transducers that have the 3D mechanical technology are: 7VC2 and the 9VE4 4D ultrasound is the sequential stacking of B mode and color slices over time – to give a real-time display of 3D volume data. This technology allows for the visualization of a 3D baby face in real time. 4D ultrasound can not be performed with a freehand acquisition. There are 2 different technologies for 4D ultrasound. • Mechanical 4D is when the transducer motor moves the crystal and sound beam back and forth consistently to produce a real time 3D volume. The transducers that have the 4D technology are the 7VC2 and the 9VE4 • Electronic 4D in full-volume 4D ultrasound, is all elements of the transducer are fired in sequence and the full aperture of the array is utilized. The transducer that has this technology is the 5Z1. Measurements can only be performed on mechanical or electronic sweeps as these methods are more accurate than a freehand movement. This area represents the entire scan area. The VOI (Volume of Interest) graphic appears as a graphic outline made up of a solid green line and corner icons and defines the content of the rendered volume. The 3D and 4D function keys are available and the active function and will be backlit in orange. 1. 3D: Activates the 3D imaging function in order to perform an automatic sweep 2. 4D: Activates the 4D imaging function in order to view a real-time 3D image 3. Surface: Smooths the image contours, creating a soft sculptured appearance for highlighting surface features. Will use the mode specific selections for opacity percentage and threshold 4. Vascular: (Minimum Intensity Projection) limits the display to minimum-value pixels with the lowest value along each projection of the volume It is a helpful tool when visualizing hypo-echoic lesions and large vascular structures - e.g. adnexa area 5. Skeletal: (Maximum Intensity Projection) limits the display to maximum-value pixels with the highest value along each projection of the volume. May be useful for body structures in obstetrical imaging and intrauterine devices in gynecological applications 6. Light Source: The moveable light source helps increase depth perception, reveal surface details and can provide a deeper understanding of relational anatomy 7. Reset Curve: Resets the curve of the VOI 8. VOI: Enables or disables the VOI. In acquisition mode this allows the entire field of view to be acquired. Available values are Low, Mid and High. • High - scan line density with the highest setting enhances the detail resolution of the rendered volume, but decreases the volumes per second during acquisition • Mid - scan line density enhances the volumes per second during acquisition. This results in a limited loss of volume resolution • Low - scan line density has the lowest rendered volume quality, but the highest volumes per second during acquisition Angle: The volume angle determines the distance of the volume sweep. The options and layout will be different from the 3D/4D mechanical and the freehand transducers. The Touch Screen will be similar to the Cardiac workflow. The 5Z1 transducer only has 4D imaging and does not have a VOI option. Therefore, when the 3D/4D option is selected, it will launch directly into 4D. If the user needs a 3D volume, select freeze and then select the image key. VOI workflow can be achieved either by selecting 4D Zoom or the Single V option from the Touch Screen. 5Z1 features BiPlane+ which will allow high volume frame rates and will be discussed later in this presentation. The current GI volume manipulation tools will not be available on the 5Z1 including FlexPlane, Multislice or Editing functions including animation. The acquisition image is on the left and the image on the right will be the BiPlane+ image which is 90° from the acquisition plane. The Angle rotation is located on the soft keys and is displayed at the top of the image to indicate the amount of rotation for each image. The angle rotation will default to 1° increments. Press the Angle Rotation knob to change the increments to 90° when rotating the knob. Please Note: The 5Z1 workflow is the same as the cardiac workflow, therefore there will be inconsistencies for the new GI workflow. The 5Z1 is a cardiac transducer, therefore scanning in cardiac exams, the transducer rotates to the right, whereas in general imaging, the transducer rotation is to the left. Therefore, it is important to know why the Angle rotation is correct depending if the acquisition scanning is in the Sagittal or Transverse plane. We will start with the sagittal scan. When scanning in general imaging 2D, the user will have the transducer notch towards the patient's head, therefore the transverse plane will be 90° to the patients right. In the BiPlane+ feature, the image on the right should be the transverse image or 90° to the patients right. Although it’s important to remember that the rotation of BiPlane is to the right, not the left, therefore it will need to rotate 270° until it reaches the correct orientation. Therefore, when using BiPlane+, the image on the right should be set to 90° as the acquisition plane is 0° and it rotates to the right which would only need 90° to be at the correct orientation. In Summary, when scanning in sagittal, be sure the orientation is set to 270° and when scanning in transverse, be sure the orientation is set to 90°. For example, if you are in a sagittal plane with a 270° angle rotation and then switch to a transverse plane, instead of manually changing the rotation to 90°, select Right Invert key and the image on the right will change to be in the correct orientation. This will also work when you are in a transverse plane at 90° and go to a sagittal plane and need the 270° rotation. When using the BiPlane feature, orientation is important. The user must set up their preferences as mentioned in the previous slides. Tilt Angle represents the right view port angle, and this can be used to propagate in a perpendicular plane to the anatomy (as seen here with the transverse uterus). To move the MPR line place the pointer along the line until it changes from a solid line to a dotted line, then with the trackball move the line. The user may toggle between Free Plane and Rotate by using the trackball. Free Plane is not connected to any MPRs or specific cut planes. The user can place this anywhere. To rotate the Free Plane in X and Y select the first Soft key. HD Zoom is available by selecting the 3D tab or during live 4D. When BiPlane is selected, a dual screen will appear and the ROI can be sized and positioned axially, laterally and elevationally. To enter HD Zoom, select Update from either the Update key or the Touch Screen 4D Zoom. If the Cut Plane is needed, select it from the Touch Screen. This will be the Cut Plane that is synced to the MPR that is active.
